Social Prescribing Programme
The Social Prescribing Programme helps older people address social, emotional or practical needs by linking them to sources of support and activities within their local community. The project aims to take an early intervention approach to the frail, elderly population ages 65+ who may be experiencing personal difficulties i.e. social isolation, bereavement, a family crisis or chronic loneliness. The programme aims to enhance older people’s health and well-being to help them live as full and healthy a life as possible. Referrals must be made through a GP or other Allied Health Professional. The Stepping On Falls Prevention Programme
The falls prevention programme offers exercise classes for older people which can help to reduce falls and at the same time increase self confidence in situations where they are at risk of falling. Stepping On presents an evidence based exercise programme called Otago, which aims to strengthen muscles and improve balance, which helps to reduce falls. For men and women 65+, who have experienced a fall or who are vulnerable or at risk of a fall. For more information contact Clare Cooper on 02871 365330.
